Start with a single spreadsheet, or even a sheet of paper. Record the request date, the delivery date, hours spent, and whether the job came back for fixes. After a month you have a picture. After three, you can compare and adjust. What does not get measured does not get fixed.
A simple illustration: a design studio measures a twenty-day delivery cycle and discovers half of it is spent waiting for client approval. Adding one clear approval step shortens the cycle with no new hires. The information was always there; nobody was reading it.
